His past medical history was also significant for peripheral vascular disease and protein C deficiency for which he was on chronic anticoagulation with warfarin. Seven months prior to admission, our patient received a 5-week course of intravenous (IV) cefepime and oral metronidazole for osteomyelitis of the foot. Our patient was a 62-year-old white man with poorly controlled diabetes who was admitted to the hospital with a right great toe osteomyelitis and associated Staphylococcus aureus bacteremia secondary to diabetic foot infection. 3 This report aims to further increase awareness of the potential for DOAC-induced hypersensitivity reactions and encourage pharmacovigilance with this medication class. 1 In their literature review, Carli et al were able to identify 29 case reports of patients with possible hypersensitivity reactions to DOACs. 2 Additionally, 2.1% of the patients who received rivaroxaban during approval trials have experienced pruritus. ![]() ![]() Drug-induced hypersensitivity reactions to apixaban, including skin rash, were reported in the package insert in <1% of the patients in approval trials. With increased use of DOACs over the past decade, there have been increasing reports of adverse reactions, including hypersensitivity reactions, associated with their use. Apixaban and rivaroxaban are DOACs indicated for treatment and prevention of venous thromboembolism and stroke risk reduction in patients with nonvalvular atrial fibrillation. Additionally, DOACs do not require frequent lab monitoring and have fewer drug–drug interactions, making them an appealing therapeutic alternative to VKA. A newer class of medications, direct oral anticoagulants (DOACs), is increasingly being recommended over VKA due to fewer bleeding-related complications. Warfarin is a vitamin K antagonist (VKA) that is commonly used for multiple thromboembolic conditions requiring anticoagulation. Additionally, this report documents to our knowledge, the first report of an immediate hypersensitivity reaction to DOACs. Future research is needed to further investigate this adverse event and ensure that providers and patients are aware of it. This case report indicates a potential rare adverse drug reaction that may become more prominent given the increasing utilization of DOACs in clinical practice. After 3 days of DOAC therapy, the patient was reverted back to warfarin and within 24 hours the rash subsided. The following day, apixaban was replaced with rivaroxaban, while the rash continued to worsen and spread further around his back. Within 6 hours of receiving the first dose of apixaban, he developed a hemorrhagic pruritic rash around his buttocks area and extending into his groin. During hospitalization for another issue, his anticoagulant was converted to apixaban. We describe a case of apixaban-induced hemorrhagic pruritic rash in a 62-year-old man with protein C deficiency for which he was receiving warfarin. While DOACs have fewer associated adverse events as compared to warfarin, DOAC-induced hypersensitivity is a rare adverse event that has been reported in the literature. You can review the footage and even see where the drive took place on a map (this is why the app requires location). It will also tell you how much storage is already in use by Nexar video footage. The app will tell you how many gigabytes of space is reserved on your phone, which equates to hours of footage. The Nexar app allows you to delegate a certain percentage of your available phone storage just for Nexar video, and this can be 20, 50, or 70 percent of your remaining storage. Since the Beam connects with your phone and saves footage to your phone in addition to the memory card, you'll want to make sure that there is space on your iPhone for video. ![]() Nexar App Drive Summary (Image credit: iMore) I do this each time to make sure that I don't miss a thing, plus Nexar states that the auto-start can take up to four minutes, though they're working to improve that in the future. To ensure that this always happens, you can manually connect it each time you drive by connecting to the "Direct-Nexar" network in your iPhone's Wi-Fi settings. You'll want to make sure that the blue LED light is solid, though, not blinking, because that confirms that it's "paired" with your phone and recording has begun. The whole process takes no more than a few minutes, and once it's set up, the Nexar Beam should always automatically connect to your phone as long as the Nexar app is running in the background. You just need to select the Nexar Beam in your iPhone's Wi-Fi settings, input the password (12345678), set the Location use to "Always," allow Bluetooth when requested, and then it's paired. The setup process for connecting the Nexar Beam to your iPhone is pretty easy and straightforward. The length of the cable is nice because you can string the cable around the edge of the windshield, passenger door, and under the dash and floor cover. Nexar also includes a nice, 3-meter long micro USB cable with a car charger, which you can plug into your car's cigarette lighter port. The mount slides into the top of the camera and clicks in place, so you know it's secure. With the Beam, it includes a suction cup mount that easily sticks to your windshield and locks in place. Along the left side of the camera is the power button, as well as a slot for a microSD card, which is included for free (32GB). There isn't anything on the back of the camera except the Nexar logo and two LED lights to indicate that it is recording (green) and paired to your smartphone (blue). ![]() (Of course, none of them notices that Yellow Suit Guy totally overheard them.
